Welcome to this week's "Last Week Now", the newsletter that summaries all the news from last week, now (see what we did there?)

Last week we talked about the future of Emergency Medicine, chest pain in pregnancy, shock revisited, and steroids in community acquired pneumonia.

The time to start our review is here, so let's get ready to see what happened Last Week, NOW!

The 2023 Emergency Medicine match was a disaster. Over 550 spots went unfilled and many have said this is the end of Emergency Medicine. I brought on Dr. ChriHtopher Doty who is the Vice Chair of Education and my former program director to look at the facts (and the fiction) from the match. Although there are many things to be concerned about, don't count EM out just yet.

Check out my entire interview with Dr. Doty here.

Steroids in Community Acquired Pneumonia

There is no shortage of controversy with regards to critical illness and steroids. One of the most popular controversy is steroids in pneumonia, specifically community-acquired PNA. It makes sense that steroids would help to mediate the inflammatory response, but the data has been conflicting.

A recent trial in the NEJM called CAPECOD evaluated steroids for CAP and found benefit....but is that the complete story? The ICU-CAST, a podcast that I recently discover, took this trial head on and breaks it down. The hosts take a look back at steroids in critical illness in general and then focus on steroids in CAP as was evaluated in this study. These podcasters were both entertaining and informative and will definitely  be checking out this new FOAMed podcast. Here's the episode on CAPECOD.

Have you had a pregnancy patient with chest pain before? It ain't easy. The diagnosis is probably benign, but you must stay vigilant as there are life-threatening diagnoses to consider. This week, we have world-class educator Dr. Tarlan Hedayati to break it all down, from S.C.A.D. to pulmonary embolism. This is a great review and approach to these patients.

This episode is available in both audio and video format so head over and check out this episode now!

Dr. Sara Crager is both an Intensivist and Emergency Medicine physician with a strong focus on resuscitation. In this episode Dr. Crager breaks down the classic definition and teachings of shock and offers an alternative viewpoint and approach. Sara has spoken at ResusX many times before as she is an exceptional educator.

This episode is a must-listen for anyone who acutely manages patients in shock. The ResusNation Podcast is FREE for All-Access Members.
