Been a bit under the weather but got some fresh content for you this week, take a look below:

  • A TikTok on the combo of Vanco & Zosyn...bad for the beans? Maybe not
  • A Tweetorial on lung ultrasound
  • The Trauma Team Leader by Scott Weingart
  • A great podcast on HALO (High Acuity Low Occurrence) procedures by Sara Gray

There’s no time to waste, it’s time for Last Week Now!

Vanco and Zosyn used to be part of a health sepsis diet...but then came data that the combination increases the risk of nephrotoxicity. But a recent study brings the presumed nephrotoxicity into question. I break it down in my series TL;DR (The Literature Dissected & Reviewed)

Lung Ultrasound 101:

In a recent tweetorial, Jonny Wilkinson shared a quick guide on how to perform lung Point of Care Ultrasound (POCUS), emphasizing its importance as a first learning modality for beginners. He highlighted the benefits of lung POCUS, stating that it is easy to perform, accurate, sensitive, repeatable, and eliminates the need for irradiation or transportation. Wilkinson also mentioned that they are teaching doctors, physiotherapists, and outreach nurses to use this technique to benefit their patients. The tweet also included a discussion about the scope of function and prescription abilities of Critical Care Outreach Teams (CCOT).

In this ResusX video, Dr. Scott Weingart discusses the role of the trauma team leader in emergency medicine and the importance of proper training for this role. He introduces a taxonomy of skills and actions that a trauma team leader needs to know. The "zero point survey" is highlighted as a way to ensure team cohesion and preparedness. The trauma team leader stands at the foot of the bed to maintain control and create a calm environment. Closed-loop communication and regular sit reps are encouraged. The podium nurse plays a crucial role in coordinating tasks and equipment. The primary survey, including assessing airway, breathing, circulation, disability, and environmental factors, is discussed. Situational control and handling agitated patients are emphasized. A team approach to airway management is recommended, along with using checklists. Monitoring parameters like saturation, end-tidal CO2, and temperature are important. The trauma team leader identifies signs of shock and potential sources of bleeding through a systematic assessment. Procedures such as chest decompression, vascular access, blood transfusion, and sedation are covered, along with considerations for transfusion devices and blood products. Stabilization techniques for pelvic and femur fractures are also mentioned. Watch for an overview of the responsibilities and skills required of a trauma team leader in critical care and emergency medicine.
