Are you intubating someone who is hemodynamically unstable....don't forget about ZEEP! ZEEP, or zero PEEP can be a good inital strategy with patients who are mechanically ventilated and hemodynamically unstable. This is not a long term strategy, but consider this to minimize positive intrathoracic pressure and to maintain pre-load.

Epinephrine in Arrest

Embracing Lactated Ringers for Critical Resuscitation

Please keep in mind that the term “lactic acidosis” does not equal pathologic state! It is essential to clarify that lactic acid (H(+) + Lactate(–)) does not exist in significant quantities in the human body. Instead, lactate(–) is the primary form, with the hydrogen ion dissociated at physiologic pH (when we start to get to an actual acidotic pH then you can call it that). This distinction is crucial as it changes the perspective on lactate's role in critical illness. Increased lactate production is associated with hypoxia and perfusion abnormalities, but it may also be a normal response under stress to maintain adequate energy sources and support various physiological functions.
Lactated Ringers (LR) contain sodium lactate NOT LACTIC ACID, and evidence shows that it does not significantly raise lactate levels when compared to normal saline (NS) infusions. Furthermore, lactate in LR offers multiple physiological and clinical benefits, including increased pH from bicarb buffering, energy substrate support, improved hemodynamic stability, increased cardiac index, and increased oxygen delivery. Embracing the sodium lactate in LR provides a valuable addition to fluid resuscitation, making it a preferred choice for balanced and pH-guided resuscitation in critically ill patients.
