Pulse Oximetry Pitfalls

Pulse oximetry is a valuable non-invasive tool for continuous oxygenation monitoring, but in certain situations, its accuracy can be questionable. Historically calibrated for white patients, pulse oximeters may provide inaccurate readings in individuals with darker skin. Recent studies revealed that black patients are 3x more likely to experience occult hypoxemia when relying on pulse oximetry readings alone! Hemoglobinopathies also pose a pulse ox pitfall with carbon monoxide (CO) poisoning yielding falsely high readings, while methemoglobinemia can yield consistently low readings (typically around 85%). Any hypoperfusion state can also lower accuracy, as the tool relies on strong arterial pulsations. Lastly, when readings fall below 80%, pulse oximetry accuracy significantly diminishes, potentially underestimating hemoglobin saturation during profound hypoxemia. Keep these pitfalls in mind next shift so you can determine whether that finger sticker really is accurate and when in doubt, get an ABG!

 

CO poisoning = Hemoglobin binds way too tightly to CO leaving less space for O2 to jump aboard leading to less O2 to deliver to tissues

 

Methemoglobinemia = Methyl group binds to Hemoglobin molecule decreasing the amount of O2 able to be bound
