VEXUS
In this podcast episode, Dr. Abhilash Koratala from the Medical College of Wisconsin provides a comprehensive overview of VEXUS (Venous Excess Ultrasound), a valuable tool for assessing a patient's hemodynamics and fluid status at the bedside. Dr. Koratala explains the three key components of evaluating the hemodynamic circuit, which include Forward Flow, Fluid Responsiveness, and Backward Pressures of the heart. He emphasizes the importance of assessing Systemic Venous Congestion and its impact on organ perfusion, particularly the kidneys. VEXUS combines various ultrasound parameters, including Inferior Vena Cava, Hepatic Vein, Portal Vein, and Intrarenal Vein, to quantify venous congestion and predict the risk of organ injury. The podcast also includes practical insights and case examples of how VEXUS can guide treatment decisions and monitor patient response to therapy.

Post-Arrest Interventions
In this podcast segment, Dr. Mike Winters discusses the importance of Post-Arrest Interventions following the successful return of spontaneous circulation (ROSC) in patients who have experienced sudden cardiac arrest. He emphasizes that the critical period after ROSC has the greatest impact on a patient's neurologic survival. The two main post-arrest goals are optimizing neurologic resuscitation and preventing secondary injury. To achieve these goals, Dr. Winters highlights several targeted interventions, including optimizing ventilation to maintain normal carbon dioxide levels, detecting seizures in comatose patients through EEG monitoring, determining the appropriateness of emergent coronary angiography for patients with ST-segment elevation, and considering delayed coronary angiography for patients with NSTEMI. He also discusses the need for careful assessment and re-evaluation of EKG findings in the early post-arrest period.

Talking ABOUT Delta EtCO2 in Cardiac Arrest
We all know how important EtCO2 (end-tidal CO2) can be in a cardiac arrest situation but have you ever considered assessing the delta EtCO2? A study out of Canada (Eh?) assessed the utility of a delta EtCO2 in cardiac arrest patients with pulseless electrical activity (PEA). They defined the delta EtCO2, as the difference between the initial EtCO2 recorded one minute after advanced airway placement and the final EtCO2 value one minute before return of spontaneous circulation (ROSC) or at the termination of resuscitation, was investigated.The study focused on nontraumatic PEA cases and analyzed data from 208 cardiac arrest patients treated by Paramedics in two Ontario regions from January 2018 to December 2019.
Interestingly, the study showed a positive linear relationship between delta EtCO2 and subsequent prehospital ROSC, with an odds ratio of 1.74 for every 10 mmHg increase in delta EtCO2. A delta EtCO2 of >20 mmHg had a high specificity of 95% in predicting future ROSC events. Keep in mind, this data set is retrospective and in a prehospital setting, however, the findings do suggest that monitoring changes in EtCO2 during resuscitation can serve as a valuable prognostication tool for PEA patients, emphasizing the importance of delivering high-quality chest compressions and promptly addressing reversible causes of PEA in clinical practice.
