Stop It With the Useless ABGs
One of the most painful and useless test around is the ABG in an awake patient. Not only is it painful, increases the risk of complications, but it doesn't add anything to clinical care. STOP THE GUIDEWIRE FROM TICKLING THE BRAIN
In this video I discuss a complication that can occur during central line placement – when the guidewire unintentionally advances towards the brain. Despite the initial shift towards ultrasound-guided procedures for internal jugular veins (IJ), subclavian vein lines are still being utilized, putting at risk for this complication. To tackle this issue, the episode introduces the "ambash maneuver," a technique that involves gently including the ipsilateral internal jugular vein while passing the guidewire, redirecting it away from the brain and towards the mediastinum. It's essential to exercise caution and avoid compressing the carotid artery during this maneuver for patient safety. Overall, the episode underscores the benefits of using ultrasound guidance for subclavian lines to enhance safety and precision in central line placement.
