Stop It With the Useless ABGs 

One of the most painful and useless test around is the ABG in an awake patient. Not only is it painful, increases the risk of complications, but it doesn't add anything to clinical care. Serious, it's useless. Peri-Arrest in the Crashing Patient

In this episode, Dr. Mohammed Hagahmed, a faculty physician at the University of Pittsburgh Medical Center, shares a compelling case of a patient named Miss Z, a recent immigrant with COPD. A Cuff Above

Ever wondered why the cuff pressure in endotracheal tubes (ETTs) matters during mechanical ventilation? Some of the reasons for underinflation should be straightforward like avoiding complications such as micro-aspiration/ventilator-associated pneumonia (VAP) and maintaining a seal for positive pressure ventilation without volume loss. But how does one figure out what the proper volume should be in your tube? Over the years there have been various devices used such as the Posey Cufflator, Pyton, and AG Cuffill. These devices help regulate cuff pressures within the recommended range of 20-30cmH2O. 

Now we know why too little air in the cuff is important but what about overinflating? Complications arising from improper cuff pressures range from tracheal wall damage and ischemic injury to inflammation and potential post-extubation side effects like hoarseness and stridor. One of my personal experiences with this was during the COVID Delta Wave in the ICU in residency. Some of these cuffs ended up being way overinflated due to worsening cuff leaks on the COVID long haulers (50-60cm H20!). This would lead to cuffs popping above the vocal cords! Tracheal wall injury and extreme driving pressures most likely caused this. So every time I rounded I would make sure to go up to the patient and listen for breathing sounds coming from their mouth (should never hear) to recognize cuff leak from an ETT that slipped above the vocal cords due to such high pressures from severe ARDS. After intubation, you should ALWAYS check for cuff leak! Remember, cuff pressures may seem intuitive but you wouldn’t want to miss this in a critically ill patient!
