Resuscitate Before You Intubate

If you want to avoid post-intubation hypotension or cardiac arrest, then you need to watch this video. It discusses some of the key elements that puts patients at risk and the steps you can take to reduce the risk

Check out the video here.

End-Tidal [Part I]

In this episode, Dr. Max Hockstein, an Emergency Physician and Intensivist, delves into the intricacies of End-Tidal Capnography. The discussion covers the terminology surrounding this topic, its significance in understanding Minute Ventilation and Cardiac Output, and the concept of VQ matching in the lungs. Dr. Hockstein explains the working principles of modern End-Tidal measurement, including Non-Dispersive Infrared Absorption, and distinguishes between sidestream and mainstream Capnometry. The episode explores the phases of a Time Capnogram and the utility of End-Tidal in various medical scenarios, from intubation confirmation to resuscitation and preload responsiveness. The episode offers valuable insights into this critical aspect of medical monitoring and leaves listeners eagerly anticipating act two.

ADVANCED CARDIAC MONITORING

This tweet by Mark Ramzy, DO, EMT-P (@MRamzyDO) is about advanced cardiac measurements using point-of-care ultrasound (POCUS). This is the first in a three-part series and discusses various methods for assessing the left ventricle. These methods include:

1. MAPSE (Mitral Annular Plane Systolic Excursion): A technique to evaluate left ventricular function.
2. Lateral and Septal Annular Velocity: Assessing the movement of the heart's annular parts.
3. Mitral Inflow Velocity (E/A ratio): A measure used to evaluate diastolic function of the heart.

The Great Trauma Debate

In this episode of "The Great Trauma Debate," the two experts, Dennis Kim and Chris Hicks, discuss the topic of who should own the trauma resuscitation (resus) process. The debate is lively and humorous, with both participants providing strong arguments for their perspectives. Dennis Kim argues that trauma surgeons should own the resus process for several reasons, including their role as intensivists, technical proficiency, and the ability to provide definitive care for life-threatening injuries. On the other hand, Chris Hicks contends that the question of ownership is not as important as collaborative teamwork. He highlights the importance of trust, mutual respect, and understanding between trauma surgeons and emergency physicians, emphasizing that the model that works best depends on the specific healthcare system in place.

Key Takeaways:

  1. Trauma Resuscitation Ownership: Dennis Kim argues that trauma surgeons should own the trauma resuscitation due to their expertise, experience, and ability to provide definitive care for critically injured patients.
  2. Collaborative Teamwork: Chris Hicks emphasizes the importance of collaboration and a team approach in trauma resuscitation, where the choice of ownership may vary based on the specific healthcare system.
  3. Non-Technical Skills: Both participants acknowledge the significance of non-technical skills like communication, effective leadership, teamwork, and situational awareness in managing trauma resuscitations.
  4. Role of Trust and Mutual Respect: A trusting and respectful relationship between trauma surgeons and emergency physicians is crucial for effective trauma resuscitation.
  5. System-Specific Models: The choice of ownership may depend on the healthcare system's structure, with some systems favoring a collaborative approach, while others may lean more toward one discipline taking the lead.
