Looking Ahead Towards 2024
I want to personally thank you for making 2023 an amazing year for me. I started this newsletter thinking only a few hundred people will join, but we close 2024 with over 20,000 subscribers! We are also adding a number of things to the newsletter to make it even more helpful to you in your daily care of critically ill patients.
2024 will also be the year that we plan more educational activities including a weekly YouTube show, more conferences, more UltraRounds conferences and our first airway course. I'm excited to unveil all of this to you all very soon.
For now I wanted to thank you for your support, for your kind words, and most importantly for your determination to help the sickest patients in the hospitals. This newsletter is dedicated to each and everyone of you. Thank you all and Happy New Year!

Sharpen Your BMV Skills!
There's a way to perform BVM and there's the best way. In this video I duet @sangrialovingairwaydoc on IG as he teaches a trainee on the proper way to perform a BVM for the best seal and the most effective ventilations.
The comments section, didn't disappoint since this is always a hot topic of discussion. Would love to hear your thoughts on how you prefer to perform a BVM on patients.

Trauma Discussion
In this episode, trauma guru Dr. Zaf Qasim engages in a candid and discussion on trauma management with a focus on resuscitation strategies. The conversation covers various aspects, from challenging dogmas in trauma care to addressing specific scenarios like traumatic arrest. Zaf emphasizes the importance of tailoring interventions based on the unique physiology of traumatic arrest, distinguishing it from medical arrest. He delves into the sequence of procedures, advocating for a methodical approach that prioritizes addressing reversible issues. The debate extends to the use of vasopressors in trauma-induced hypotension, with a spotlight on vasopressin as a potential agent. The discussion also touches on crystalloid administration in the absence of immediate blood availability and the role of calcium in trauma resuscitation. Dr. Qasim provides insights into the evolving consensus on tranexamic acid (TXA) use, including dosing considerations, and sheds light on the integration of TXA into trauma protocols. The episode concludes with audience questions, exploring topics such as RSI in trauma, including considerations for induction doses, and the role of pericardiocentesis in trauma patients with effusion or tamponade. Overall, the episode offers a valuable blend of evidence-based practices and practical considerations for trauma resuscitation.

You Gotta Check Out These ICU Notes
You have to check out this account @ICUNursingNotesByCaitlyn. This is a collection of illustrations drawn by this phenomenal artist that contain high-yield information for nursing (or any physician) trainees who are rotating in the ICU. I've been scrolling through her channel for days now and we even collaborated on a post here. I wanted to share this with all of you because she is an amazing educator that y'all need to follow.

Last-Line Vasopressors
In this podcast episode, Dr. Terren Trott joins the host to discuss the critical topic of last-line vasopressors in septic shock. Dr. Trott emphasizes the challenges faced when standard approaches fail, leading to refractory septic shock in critically ill patients. He introduces four potential last-line interventions: angiotensin 2, IVIG (intravenous immunoglobulin), methylene blue, and esmolol. Dr. Trott reviews the rationale behind each intervention, citing studies and evidence to support their consideration. The discussion includes the risks associated with these last-line options and the importance of carefully assessing each patient's condition before implementing them. The key takeaway points include the potential benefits of angiotensin 2 in refractory shock, the controversial status of IVIG with weak recommendations against its use in sepsis guidelines, the promising role of methylene blue in inhibiting nitric oxide production, and the favorable outcomes seen with esmolol in reducing heart rate and mortality. Dr. Trott encourages a bedside approach, emphasizing the need for careful consideration and monitoring when exploring these last-line interventions in the critical care setting.
Key Takeaways:
- Angiotensin 2 shows promise as a last-line intervention in refractory septic shock, providing a potential multimodal approach to vasopressors.
- Intravenous immunoglobulin (IVIG) remains controversial, with conflicting evidence and a weak recommendation against its use in surviving sepsis guidelines.
- Methylene blue's inhibition of nitric oxide production presents a rational approach to improving vascular tone in septic shock, supported by positive outcomes in studies.
- Esmolol's short duration of action and benefits in reducing heart rate make it a viable option in septic shock, with studies demonstrating improved mortality and other positive outcomes.
- The bedside clinician is encouraged to judiciously explore these last-line interventions, considering the specific needs and responses of individual patients in the critical care setting.
